To successfully secure benefits we provide a comprehensive end-to-end service designed to mitigate the risks associated with the MassHealth Process. This includes filling out the MassHealth Long-Term Care application, submitting application and supporting documents, communicating with the facility and MassHealth on your behalf.

As of October 2025 MassHealth has mandated a comprehensive 60-month (5 year) look-back period for all asset accounts.
Applicants must provide financial statements covering the full five-year look-back period. While MassHealth may historically have accepted 1–2 years of records, current enforcement requires verifying every deposit and any withdrawal typically over $1,000.
The state maintains the right to question any withdrawal of any amount, particularly those that close an account. Documentation such as check images, invoices, or receipts is required to prove the funds were not gifted for less than fair market value.
Although letters of explanation can be submitted, acceptance rests solely with the caseworker’s discretion.
​In 2025, gifts or unverified transfers made within the look-back period trigger a penalty period. This is calculated by dividing the total transfer amount by the state's average daily nursing home cost, which is $441 per day in 2025.
Medicaid Planning Group provides specialized navigation for these penalty periods, utilizing spend-down techniques (e.g., quality of life preservation or medicaid annuities) to ensure residents are not discharged from facilities due to pending eligibility.
SERVICES
Asset Assessment - $500 Review all assets owned by you or your spouse. Explain how MassHealth will view them. Provide a plan to protect them.
Banking Review - $2,000 MassHealth goes through a detailed financial audit of the last 60 months of assets. You must provide all pages of the last 60 months of statements for all assets including but not limited to any bank account (checking, money market, cd, savings, etc.), 401K, IRA, annuities, stocks, etc. Any withdrawal is subject to questioning, but typically they focus on $1,000 or more needs to be explained with documented proof. Any deposit needs to be explained with documented proof.
Common transactions that could effect the eligibility include gifting car for $1, helping family members
with college, taking out monthly lump cash withdrawals for petty cash on hand.
We will do an audit of your finances to find any bank patterns that could effect your eligibility and prepare a solution.
Long-Term Care application - $5,000 We will complete all necessary applications and supplements for your eligiblity needs. We communicate with MassHealth and the nursing homes. We will help you gather all the necessary documentation MassHealth looks for. We will integrate all documented proof of all questionable transactions within the banking so we submit a clear picture of your finances for MassHealth to review. This also includes basic appeals like failure to verify, over assets or wrong patient paid amount calculations,
Appeals - $500 - $1,000 Appeals not included in application process like penalty periods or termination notices.
Reviews $500 MassHealth conducts yearly reviews to verify changes to income and health insurance premiums. They also verify you have maintained your assets below $2,000.
Asset Split - $500 - $1,000 Once a married couple have proven they are below $159,920 they must split their assets. The spouse in the nursing home can only have $2,000 in their name. The healthy spouse or community spouse may keep the other $157,920. We will work with you to accomplish this and provide proof to the state.

Eligibility
A single individual is eligible when all their assets are below $2,000.00. A married couple is eligible when all their combined assets are below $159,920.00. When you leave our consult you will have a step-by-step guide to reduce your assets without jeopardizing MassHealth benefits. You also receive a detailed list of documents needed to process the application. Failure to provide the correct documentation to the state will result in a denial.
Yearly Reviews
After an individual has received MassHealth the family will have to complete a yearly review. We can either complete reviews on your behalf throughout your MassHealth eligiblity OR we can teach you how to complete a review.
